This memorandum forwards the subject DOE technical standard, DOE-HDBK-1139/3-2004
which has been approved for publication and distribution. This standard was reviewed by
the Cognizant Secretarial Officer organizations and Operations Offices through a formal
coordination process and all comments have been appropriately resolved. Additionally, the
Office of Worker Protection Programs and Policy has reviewed and approved the document.
A camera-ready copy and an electronic version of the standard in Microsoff Word format
